Consumer Confidence Report Request Forms
The 1996 Safe Drinking Water Act Amendments require all Community Water Systems to prepare an Annual Drinking Water Quality Report, also known as a Consumer Confidence Report (CCR) each year, and submit it to your state primacy no later than July 1st. A CCR describes the quality of your water, your source of supply, testing results, and violations.

Please also include/mail to the office, if you have not already:
- 2018 Membership Dues,
- 2017 CCR Fee,
- 2017 Laboratory Data,
- All 2017 Public Notices and/or Violations you received from CT DPH, if applicable.

2017 RI CCR Request From (Testing year 2017)
Click here to downloand the Request Form. Please complete the form, sign and date it, then submit it to our office at your earliest convenience, however, no later than Monday, April 30, 2018.
We are pleased to announce that the State of Rhode Island Department of Health has again contracted with us to prepare your water system’s CCR for the 2017 testing year. Therefore, all Community Water Systems serving less than 10,000 population, you are eligible to receive a free report. All you need to do is complete the form and send it to our office.
Please Note: After June 30th, you will not be eligible to receive a free report under this program. ASRWWA will be happy to assist you in the completion of your CCR for a fee.
